Additional information: Recruiting for 1 permanent position - Interviews will be early January _____________________________________________________________________________________ This post will support the Independent Living team at the Watchfactory, Crawford Gardens and our sheltered housing properties. _____________________________________________________________________________________ The successful candidate will be expected to undertake a DBS check and have the appropriate clearance to the role

To provide ongoing practical, flexible and individualised support enabling clients to achieve outcome focused goals which increases independence and assist them to maintain/ access appropriate accommodation across the sheltered and extra care schemes.

 

  • Responsible for assisting clients in settling into their home, maintaining their independence and reducing social isolation.

 

  • Responsible for the initial completion of an independent living assessment for the client to assist in increasing independence

 

  • Responsible for identifying new clients to the service, generating sales and meeting monthly personal sales targets

 

  • Responsible for marketing any supported housing schemes, encouraging potential clients to visit the scheme, showing accommodation to applicants and leading in promotional activities.

 

  • Provide advice and /or signposting to relevant services that can help achieve income maximisation and act as advocate on their behalf with financial bodies.

 

  • Resolve low level neighbour disputes and environmental issues

 

  • Responsible for health and safety of their work area including regular checks of assistive technology systems, inspections of communal areas and monitor the delivery of facilities service contracts

 

  • Attending multiagency meetings as required on behalf of the business

 

  • Ensure that our Customers are at the heart of everything we do.
  • To read, understand and demonstrate a commitment to the Group’s Employee Charter.
  • Expected to ensure compliance with all legal obligations concerning the protection of data concerning employees, customers and third parties.

 

  • Adapt a proactive approach to identify and mitigate risks to your business area by informing your line manager and contribute to updating the risk register

 

 

  • To take responsibility for your own health and safety and that of your colleagues in accordance with the Health and Safety at Work Act (1974), whilst following the Group’s Health and Safety policies and procedures at all times.

 

  • To work in a variety of locations including premises not directly under the control of the Group

 

  • Some flexibility over working hours is necessary to meet the requirements of the job.

 

  • Customer outcomes are recorded showing improved independence

  • Evidence of attendance at multiagency meeting through minutes and attendance list
